As regards initiative, you're right that it could be quite deadly. As Narrator/GM you can choose to not have the adversaries have their shots set to kill, especially if they're faster than the PCs....
I've now run it a couple of times and it is a very well done system and quite fun. Very different in many ways (more narrative than crunchy), but it really captures the feel of Tolkien's world.
