Nice, simple, clean. I like it.
A few editing things, though.
You didn't leave a space for "Atmosphere Capable".
The shield damage track "A...System Offline: Vessel Blind" doesn't make much sense
You need to insert a space in the Life Support damage track "E...Random Console Explodes: 1d6stun"
You need to even out the spacing on your yellow data "bubbles".
Still, it's an ellegant, simple form, which looks great. Just a little tweaking to do for the final draft, methinks.

Chris, thanks for bringing these things to my attention. Some of them I saw myself after posting the form and all of them will be corrected in Version 1.1.

Quick question about the Structure tracks - If I click on one of the boxes, all 5 for the group get checked. I'm not fully at home with the rules yet, but is this right?

This is intentional, as the NG says you can convert space to structure and vice versa in blocks of 5 each and edges/flaws for ships cost/give 5 spaces each.Originally posted by BonzaiGopher

Don said elsewhere that this is a hard rule and not just a guideline, as each 5 points of structural damage lead to systems damage. So you shouldn't have a ship with structure that's not evenly divisible by 5.
However, if you're using the sheet to help in combat, then you need to be able to select one point of damage at the time. :-)
