If you have a computer next to you during the game and want to click form fields instead of printing the sheet and using a pencil just go ahead and change the form fields.
All you need is Acrobat Writer to give each structure box a different name and hey presto! - one point structure fields.

    Yup. :-)

Yep, that's what I did called the fields X{a,b,c,d,e} and voila.

BTW, what's you use to get the LCARS look? I'm trying to play around and make some of the other forms in the NG, and can't quite figure it out.

Oh I don't use any fancy layout program if that's what you think, but only a simple word processor (in my case Sun's 'Star Office').

Most of the LCARS boxes are just rounded text fields with colored backgrounds.
The system damage tracks were made the same way with superimposed white lines.
The "Ship" and "Class" fields are blue rounded text fields again with superimposed yellow fields. To get the with lines between the yellow and blue don't put white lines on top. Just make the outline of the yellow box white lines and adjust border width as needed.
The structure and protection tracks are yet again text fields, this time with a slim black outline and a horizontal/vertical black grid as a background.

Maybe not the best way to do it, but it works for me.
